Observation

110-300-0455(1) Licensor reviewed attendance records of a child in care. It was observed the child was in care for more than 10 hours each day. The provider did not have an agreed upon alternate schedule documented in the child’s file. 110-300-0455(3) Licensor asked for the daily staff attendance records for each classroom in the center to review, who was in the classroom, number of children, staff-to-child ratio, the date, and the start and end times of the assigned staff or volunteers. The director was not able to locate the daily classroom attendance records for review by the department.
